Patient Intake Form
We accept referrals from physicians, families, case managers, as well as, community agencies. To refer a patient, simply print out and fax the Patient Intake Form to 713-779-9600. Once this information is received someone from our office will initiate the referral process and be in touch shortly.

MedCare Resources for Parents
At MedCare, we believe in providing more to our families than just clinical treatment. We feel that educating and uniting families is one of the most powerful resources we can offer. Below is a listing of just some of our programs which provide support and growth for families and children with special needs throughout our community.

Transportation Services:
MedCare believes in removing as many obstacles as possible from children receiving the services they need. Therefore, we offer transportation to and from all of our outpatient rehab centers for children in need. Our buses are fully equipped with all the latest technology including an entertainment system for the kids to watch movies, and are staffed with professionally-trained drivers and attendants. All buses are ADA-accessible and decorated in a fun and inviting underwater theme.

Love & Logic: A course offered throughout the year by our own certified staff to teach parentsreal-life techniques for parenting children ages birth – 6 years old in a respectful, loving environment. The course covers topics including how to handle temper tantrums, bed-time issues, and teaching children responsibility.

Parent Education & Support: Throughout the year, we offer free seminars to parents of children with special needs on such topics as finding resources and support groups, the transition from pediatric to adult care, parenting skills and more. If you’re part of a support group who needs outside speakers or perhaps a place to meet- call us- we have many specialized therapists and nurses that can address your group and we also have a conference room that can be requested for group meetings.
